What We Need From You To Complete Your Taxes

1. Copy of photo ID for all individuals filing taxes (we can make the copies in the office)
2. Copy of Social Security card for everyone on the return (including children – we can make the copies in the office)
3. All of your tax applicable information
4. ProAcct Financial forms filled out (Intake Interview Sheet, Engagement Letter and Privacy Disclosure, Bank Information sheet)
